Changing your child’s Status

To get started, all you need to do is to sign and date the documents listed below and forward them to your child’s disability office. To help increase your child’s benefit as fast as possible please sign and date the documents and forward them today!

If you receive a monthly employment or “Changes Report” attached to your child’s monthly statement, check or write in “Change of Status” to “Renting“.
